All Chapter 12 Collectibles | Mafia The Old Country

This guide covers all collectible locations you can find in Chapter 12 of the game Mafia: The Old Country, released on 8th August 2025.

Note 1/6: Isabella Plea

Note: The Isabella Plea can be found on the table near the sofa inside Enzo’s apartment.

Saints’ Card 1/1

Follow the main story until you receive the objective “Follow the priest.” On the left wall, you’ll see a small altar. Approach it to collect the Saints’ Card.

Note 2/6: Smuggler’s message

Follow the priest to reach the basement. He will instruct you to move the boxes to clear the path. Once the path is clear, you’ll find the note on a wooden barrel inside.

Charm 1/2

Proceed to the objective “Raise the Gate.” You’ll find Charm #1 lying on the ground beside the underground stream.

Note 3/6

Follow the main story until you reach the objective “Get the Royal Box.” Go upstairs to the first room, where you’ll find the note on a table to the right.

Note 4/6

Note #4 is on top of a wooden box in the next room (hall).

Trinacria 1/3

The Trinacria #1 is on a table to the left side of the hall with the guards.

Mystery Fox 1/3

Mystery Fox 1/3 can be found in a wooden box on the right side of the wall. From the previous collectible, go straight onto the raised platform, then head right to find the statue.

Newspaper 1/1

Note 5/6

Go up the ladder to find the note on the pathway opposite the door.

Note 6/6

On the second floor, follow the corridor on the right to find the note on a table near the door.

Mystery Fox 2/3

Go down the stairs to find Mystery Fox 2/3 in the left corner near a table.

Mystery Fox 3/3

Follow the main story to “Stop Don Spadaro” until you return to the same room you entered after coming in from the sewer. You will find Mystery Fox 3/3 on the left side, hidden behind a wooden separator.
