Facet of Devotion
You can find the Facet of Devotion at the end of the Dissent mission.
After defeating the boss, go up the disappearing ramp/steps at the back right of the arena.
Then go straight instead of turning right towards the mission marker.
At the end, you can find a Prismatic chest. Open it to get the Fragment.
