Witcher 3

During the “Possession” Quest you can see the shadow of the possessed man is actually the demon’s shadow unlike all the non-possessed people around him.

Borderlands 3

When you deal 69 damage to an enemy your character will chuckle (You can hear it after the smash). You’ll need to use a headphone to be able to hear it. It’s subtle enough so it’s not annoying but still attentive to detail.

In RDR2 when you get drunk the texts showing controls become distorted.

Mafia 3

Police respond differently to crimes depending on whether it happened in a rich district or in a poor district. It also affects the time police takes to respond. There are many more subtle details. If you walk into a restaurant or a store in a rich neighbourhood, the restaurant owner will get mad and call police because its a white only establishment.

Grand Theft Auto 5

GTA V Story Mode(2013) has a dog that goes to it’s owners grave every day. This is inspired from the story of Greyfriar’s Bobby, a Skye Terrier who spent 14 years guarding the grave of his owner until his death. Its a well known story in Scotland, Where Rockstar North are based.

When you play Spider-Man on Saturdays, orthodox jewish NPCs will not appear in the game because of Sabbath (day of rest/worship)

In Uncharted 4, if you zoom into the bathroom mirror in Nate’s house, he will pose in front of it.

World of Warcraft

In World of Warcraft enemy NPCs appear to have a lower level when your character is drunk.
